Come explore the exciting opportunity to contribute within a growing organization!  Pathways Home Health & Hospice is celebrating over 40 years of service in the Greater Bay Area and has an immediate job opening.  We are currently searching for a devoted After Hours Triage Registered Nurse (RN) to provide clinical and administrative direction to field staff.  

 

AREAS OF RESPONSIBILITY:

The After Hours Triage Registered Nurse (RN) is responsible for the coordination of hospice and/or home care service delivery after regular business hours to adult and pediatric hospice and home care patients, including palliative care.  This individual provides clinical and administrative direction to field staff and works from the office or home.  He or she accepts incoming telephone calls, or other electronic communication, from patients, family members, caregivers, or facility staff.  They assess any information provided and recommend appropriate nursing interventions.  The ideal candidate consults with multidisciplinary team membersas indicated to determine appropriate interventions and makes recommendations.  They coordinate with clinical providers to ensure pre-scheduled visits to patients are timely and based on patient and family need.  This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.

  

QUALIFICATIONS: 

  • California Registered Nurse (RN) license required with Public Health Nurse (PHN) preferred
  • Minimum two years of nursing experience
  • Must be highly motivated, self directed, flexible, and able to work well and participate in a multidisciplinary team setting 
  • Excellent nursing assessment, communication, and problem-solving skills
  • California driver’s license and automobile insurance with minimum Bodily Injury Liability Insurance per agency policy
  • Current CPR certification – American Heart Association “BLS Health Care Provider” required upon renewal
  • Recent experience in home care or hospice preferred
