We currently have an excellent opportunity for a Registered Nurse (RN) Hospice Case Manager.



    The Registered Nurse (RN) Hospice Case Manager is responsible for coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families.  The ideal candidate collaborates with the patient and / or primary caregiver(s) and the other members of the Hospice Interdisciplinary group (IDG) to develop and implement the plan of care (POC) for the patient. He/She will have all basic nursing skills, including but not limited to nursing assessment, current symptom management knowledge, patient / caregiver teaching and documentation along with advanced skills such as collaboration, communication, negotiation, mediation, anticipation and planning. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.


    Hospice San Francisco central team This position covers our San Francisco Central territory which may include visiting patients in the following cities: San Mateo, Belmont, Half Moon Bay, El Granada, Moss Beach, Montara



    • Current California Registered Nurse (RN) License as a Graduate of an NLN accredited nursing school
    • PHN preferred
    • Minimum of one-year experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing

    • Minimum of one year hospice or home health experience preferred
    • Good eye and passion for accurate documentation
    • Excellent clinical skills and demonstrated ability to manage time well
    • Good organizational skills and teaching skills
    • Maintains IV competency that meet per Agency guidelines
    • Positive attitude and ability to confront problems constructively
    • Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary team setting.  Excellent nursing assessment, communication, and problem-solving skills
    • Current California driver’s license and automobile insurance with minimum Bodily Injury Liability Insurance coverage per agency policy
    • Current CPR certificate AHA (American Heart Association) approved
