We have an immediate opening for a Manager of Care Management Services. 

Areas of Responsibility:  The Manager of Care Management Services is responsible for the oversight of the Geriatric Care Manager program and its Care Managers. This includes program development and Care Manager Supervision in addition to providing Care Management services to clients. Care management services include meeting with prospective clients and providing a professional assessment of the needs and desires of clients in conjunction with the client’s family, physician and other health care providers to ensure quality, appropriateness and cost effective services to clients.

The ideal candidate must be able to relate to other people beyond giving and receiving instructions, communicate effectively with other team members, handle conflict and crisis situations, able to work independently in an organized and autonomous manner, able to manage schedule independently, remain flexible and balance competing priorities, and able to evaluate and provide client care consistent with standards and regulations.

Coverage areas for this position are San Mateo and Santa Clara County.

Qualifications:  *MSW degree a must. Candidates with LCSW preferred.

*A minimum of 2 years’ experience in geriatrics and/or care management required; other professional experience may substitute. Care Management Certification (CCM or CMC) preferred.

*Experienced and demonstrated competence and comfort working with families in crisis, the frail elderly, the disabled and/or the terminally ill.

*Valid California driver’s license and proof of current automobile insurance.

*Statement of prior criminal convictions.

*Satisfactory completion of Background Check and Fingerprint Clearance.

*Excellent written and verbal communication skills required.
