Come explore the exciting opportunity to contribute within a growing organization!  Pathways Home Health & Hospice is celebrating over 40 years of service in the Greater Bay Area and has an immediate opening.  We are currently accepting applications for a dedicated Nurse Practitioner (NP).  



The Nurse Practitioner (NP), in coordination and collaboration with the assigned Director of Patient Care Services and Hospice Medical Director, is responsible for the evaluation of appropriateness of on-going hospice care to identified patients. Our ideal candidate coordinates care with other members of the hospice care team to assure appropriate plan of care and recertification documentation. They serve as a consultant to the Palliative Care team patients for appropriate transition to hospice care. This person is an adjunct to the Hospice Medical Director and serves as a resource and consultant. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.




  • Nurse Practitioner (NP) with certification in palliative care preferred
  • Minimum of one-year experience as a professional nurse within the last three years or BS degree in nursing from a program accredited by the NLN
  • Minimum of 5 years of professional nursing experience preferred
  • Demonstrated program development and teaching skills preferred
  • Must be highly motivated, self directed, flexible, and able to work well and participate in a multidisciplinary team setting
  • Demonstrates willingness and ability to work with electronic input and telecommunication devices
  • Current California driver’s license and automobile insurance required
  • Current CPR Certificate
