Pathways Home Health & Hospice is celebrating over 40 years of service in the Greater Bay Area and has an immediate opening. We are currently searching for a motivated Home Health Aide Scheduler. This position is responsible for scheduling and coordinating the daily activities of the Home Health Aides for all Pathways Home Health and Hospice offices.
AREAS OF RESPONSIBILITY:
The Home Health Aide Scheduler communicates promptly and appropriately with the Home Health Aides, patients, family and patient care staff regarding Home Health Aide (HHA) scheduling issues. He or she will assist Case Managers in coordinating patient care by obtaining HHA care plan and other information as needed. The ideal candidate prints the HHA Care itineraries and assures they are distributed/communicated to aides in a timely manner. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.
