AREAS OF RESPONSIBILITY:

Home Health Registered Nurse (RN) Case Managers are responsible for developing, coordinating and directing all aspects of the interdisciplinary care and services provided to home health patients and their families.  Case management is the provision and coordination of care for an assigned caseload of patients to assist the intradiscplinary team in attaining plan of care goals.  The ideal Registered Nurse candidate will admit patients, participate in scheduling activities and provide current visit instruction for continuity of care, as needed.  He or she assures development and updates to the plan of care, as well as verifies that Home Health Aide supervision is conducted according to regulatory and agency policy requirements.  This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.

 

 

QUALIFICATIONS: 

  • Current California -  Registered Nurse (RN) License
  • Graduate of an NLN accredited nursing school
  • Meets competencies per Agency guidelines

AREAS OF RESPONSIBILITY:

The Medical Social Worker conducts an initial comprehensive evaluation as assigned and notifies the Clinical Team Manager or Patient Care Manager, as well as other members of the health care team, of general assessment and follow up plans. He or she will perform a  psychosocial  assessment, evaluate the patient/family coping, social service needs, caregiving plans, and bereavement risk along with the family’s financial status and insurance coverage. The ideal candidate will document observations, clinical findings, problems, interventions/treatments, goals and discharge plans.  They will help clarify, counsel and explain the use of medical recommendations, services, and options offered to provide optimal social and health adjustment within one's own capacity.  This individual participates in agency sponsored quality improvement activities and performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.

 

QUALIFICATIONS:

  • MSW degree from an accredited graduate School of Social Work
  • One year of professional social work experience preferred

After Hours covers visiting and caring for hospice patients between the hours of 5pm to 8:30am Monday through Sunday. The work week and hours for this position will be scheduled by After Hours Management. Generally the hours can be flexible depending on Agency needs and your availability.

 

AREAS OF RESPONSIBILITY:

The Shared Services After Hours/Visiting LVN documents observations and interventions related to the patient’s environment, physical, mental, emotional and social status.  He or she communicates care with other members of the Interdisciplinary Group (IDG) or team, including the patient’s physician, and provides skilled nursing observation of the patient’s condition, problems, care interventions, patient and family outcomes.  This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.

 

QUALIFICATIONS: 

  • Current California Licensed Vocational Nurse (LVN / LPN) license. Must have "IV and Blood Withdraw" on discipline license
  • Graduate of an NLN accredited nursing school
  • Minimum one-year experience as an LVN within the last three years
  • At least one year of acute care experience required. Experience in hospice preferred
  • Hospice or Home Care experience preferred
  • Must be highly motivated, self directed, flexible, and able to work well and participate in a multidisciplinary team setting

AREAS OF RESPONSIBILITY:

The Hospice Aide provides in-home personal care and supportive services to Hospice patients under the direct supervision of each patient’s case manager in accordance with a comprehensive patient care plan. Personal care and services include assisting a patient in bathing needs, skin and nail care, backrub, oral hygiene, passive range of motion, linen changes, assisting with ambulation devices, toileting activities, assisting with feeding the patient; maintaining safe kitchen and bathroom; monitoring of vital signs and assist patient/family as specified by case manager or supervisor. Supportive services include comfort measures and integrative therapies such as aroma therapy, Comfort Touch©™; providing companionship and conversation with patient and family members plus activities such as reading to or sitting quietly with patient. The ideal candidate provides feedback to case manager regarding changes in patient’s physical, social, or psychological condition; and makes changes in care as directed. They assist in training individuals or families in care of terminally ill patients. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.

 

QUALIFICATIONS:

  • Current California Home Health Aide Certification
  • Interest and ability to work with terminally ill patients in a variety of settings; recent experience providing personal care to patients/clients, preferred
  • Effective communication skills with demonstrated ability to read, write and speak English
  • Current California driver's license and automobile insurance with the minimum Bodily Injury Liability Insurance coverage per agency policy

The After Hours Triage Registered Nurse (RN) is responsible for the coordination of hospice and/or home care service delivery after regular business hours to adult and pediatric hospice and home care patients, including palliative care.  This individual provides clinical and administrative direction to field staff and works from the office or home.  He or she accepts incoming telephone calls, or other electronic communication, from patients, family members, caregivers, or facility staff.  They assess any information provided and recommend appropriate nursing interventions.  The ideal candidate consults with multidisciplinary team membersas indicated to determine appropriate interventions and makes recommendations.  They coordinate with clinical providers to ensure pre-scheduled visits to patients are timely and based on patient and family need.  This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.

Areas of Responsibility:

  • In 1-on-1 environments, provides company information, reviews client needs and determines appropriate non-medical care plans as part of the admission visit and participates in re-assessment visits as scheduled.
  • The ideal candidate is available to work intermittently at least two days a week during regular daytime hours, up to 5:00 pm (Flexible hours available) and also be available to work one week a month “on-call” for after-hours admission visits Monday-Friday from 5:00 pm-9:00 pm and all day one Saturday and Sunday (you pick the days). Stand by pay, is paid whether or not there are admissions during “on-call” after-hours/weekend commitment.

This position does not provide any "skilled" services or OASIS reporting and is ideal for an RN or LVN who is; working around children's school schedules; or retired or soon-to be retired; or just wants to supplement their income.

  • This is a per diem position where need can vary. Per visit pay + mileage reimbursement.
  • Coverage areas for this position are San Mateo and Santa Clara County.

Qualifications:

  • Current California RN or LVN license.
  • Graduate of an NLN accredited nursing school.
  • Flexibility to work one week per month (weekend and after hours).
  • Must be able to pass a background clearance by DOJ and FBI fingerprinting
  • Valid California driver's license and auto insurance.

The Care Transition Coordinator will market to referral sources and potential clients; provide ongoing community outreach and education to referral sources and the community and participate in re-assessment visits as scheduled.

The ideal candidate is has a polished, professional image, possesses a strong customer service approach, is detail-oriented and has excellent verbal and written communication skill, and public speaking expertise. The ideal candidate is able to interact with a wide range of private duty customers, including health care institutions, referral sources and community organizations and is able to perform effectively in a team environment.

Coverage areas for this position are San Mateo and Santa Clara County. 

Qualifications: 

*Current California LVN license or 2 years Non-medical Homecare experience. 

*2 years Sales/Customer service experience
