He or she participates in development of in-service education and oversee quality assurance activities for the social work services. The Senior Social Worker collaborates with Bereavement Department to evaluate bereavement risk factors, develop individualized bereavement care plans, and provide individual and/or group bereavement counseling and education. They provide guidance, education and referrals to help patients and families access appropriately social services including caregiving resource, advanced planning needs, mortuary arrangements, and/or financial resource. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.   QUALIFICATION: - MSW degree from an accredited graduate School of Social Work, LCSW preferred. - Minimum two years of professional social work experience requires. - One year of management preferred. - Experience with program development preferred. - Experience and demonstrated competence and comfort working with families in crisis, the frail elderly, the disabled and/or the terminally ill. - California driver’s license and automobile insurance with minimum Bodily Injury Liability Coverage Insurance per agency policy. - Demonstrates willingness and ability to work with electronic input and telecommunication devices

POSITION SUMMARY:   The Hospice Medical Director is a Board Certified licensed physician in the State of California and Hospice and Palliative Care Certified who assumes overall responsibility for the medical component of the patient care program and for the supervision, scheduling and oversight of the Hospice Physician team and Nurse Practitioners, assesses appropriateness of admissions, certifies terminal prognosis and participates in establishing, reviewing and revising plans of care for Hospice patients; participates in development of in service education and quality assurance programs; acts as a medical resource/liaison to physicians and health care providers; and acts as a resource for and member of the Interdisciplinary Group (IDG). In conjunction with these administrative duties, the Hospice Medical Director serves as the Hospice Physician for the Sunnyvale Hospice Clinical Teams (IDG) and provides oversight for the clinical component of the Hospice patients’ care including visiting the patient and families for evaluation and assessment when appropriate.   ORGANIZATIONAL RELATIONSHIPS: - Receives direction and supervision from the Chief Clinical Officer. - Provides direction and supervision to the Hospice Physician team and Nurse Practitioners. - Coordinates and collaborates with the Director of Hospice Services for each branch. - Collaborates with the Patient Care Manager for each team. - Coordinates with the Pharmacy Benefit Management company and physicians for any issues regarding medication protocol. - Participates as a member of the hospice operations management group and of the assigned Interdisciplinary Group(s)   AREAS OF RESPONSIBILITY: - Responsible for the administrative management and direction of the Hospice Medical Director meetings. - Coordinates with the Pharmacy Services. - Coordinates with the Director of Quality Improvement, Education and Compliance to assure that policies and procedures are in place for areas within the scope of medical practice. - Assumes the overall responsibility for the medical component of the Hospice's patient care program. - Responsible for the hiring and orienting Hospice Physicians. - Supervises and evaluates performance of the Hospice Physician staff. - Assures availability of physician services to Hospice patient care staff for emergency situations in the event that the patient's attending physician is unavailable; coordinates the schedules of the Hospice Physicians to assure availability of Hospice Physician at all times (24 hours a day, seven days a week.) - Provides oversight of physician services and, in addition to palliation and management of the terminal illness and related conditions, meets the general needs of the patients to the extent that these needs are not met by the attending physician. This includes visits to patients for the purpose of evaluation and assessment of status. - For the Hospice patient, participates in the assessment of admissions for appropriateness and level of care, in establishing the Interdisciplinary Group Initial Plan of Care and in subsequent reviews and revisions per agency policy and procedure. - Certifies that the Hospice patient is considered terminally ill with a life expectancy of six months or less, if the terminal illness runs its normal course. - Acts as the Agency’s Laboratory Director as defined by the Clinical Laboratory Improvement Act (CLIA).Provides supervision of Associate Medical Directors/IDG physicians. - Arranges for coverage of duties, if unable to perform these duties, assuring that covering physicians comply with all requirements of this job description. - Establishes, with the IDG, the policies governing the day-to-day provision of hospice care and services. - Attends team meetings as the physician member of the IDG, acting as a medical resource. - Acts as a medical liaison and provides professional education to physicians, health care providers, and other organizations or individuals with which the Hospice may interact. - Assists in development of in-service education and quality assurance programs. - Performs duties of Hospice Medical Director without regard for race, religion, national origin, age, sex, sexual preference or ability to pay. - Complies with all agency compliance requirements, such as maintaining a current PPD. - Performs other duties as assigned consistent with sills and training and the mission and goals of Pathways.   ESSENTIAL PHYSICAL AND MENTAL REQUIREMENTS Physical: Must be able to sit for a number of hours, climb stairs, move to other workstations, drive to meetings and other business functions, may perform repetitive motions when using equipment such as a telephone or computer. Sensory: Must be able to hear, read, write and speak clearly in English to effectively communicate and relate with staff and business affiliations in person and via telephone. Mental:  Must be able to understand supervisor's directions and carry out responsibilities; plan, direct and control activities; perform complex or varied tasks; make generalizations, evaluations or decisions without immediate supervision; understand relevant standards and regulations.     QUALIFICATIONS/REQUIREMENTS: - Board Certified with current unlimited license to practice medicine in the State of California. - Hospice and Palliative Care Certification. - Current controlled substance Registration certificate issues by the U. The individual will identify and splits cash and non-cash Remittance Advice payments and records appropriately. Our ideal candidate coordinates care with other members of the hospice care team to assure appropriate plan of care and recertification documentation. They serve as a consultant to the Palliative Care team patients for appropriate transition to hospice care. This person is an adjunct to the Hospice Medical Director and serves as a resource and consultant. This individual also performs other duties as assigned consistent with skills and training for this position in alignment with the goals and mission of Pathways Home Health & Hospice.     QUALIFICATIONS: - License as California Registered Nurse (RN) - Certification as California Nurse Practitioner (NP). - Minimum of two (2) years of Nurse Practitioner experience. - At least one year of professional nursing experience in Hospice highly preferred. - Certification as a Nurse Practitioner by a nationally accredited nursing organization in Palliative Care preferred. - Furnishing number issued by the Board of Registered Nursing required in order to furnish medications (NPF). - An individual Drug Enforcement Agency (DEA) license issued by the United States DEA for Schedule II-V controlled substances. - Current California driver's license and automobile insurance. - Current CPR Certificate.
